When reasoning about care, separate three dimensions: access (can the person obtain care — is it available and affordable?), utilization (do they actually use it, or do barriers/mistrust keep them away?), and quality (once in the system, is their care equivalent to others'?). Identifying which dimension a passage describes is a high-yield skill — and it is this guide's core scope.
Care access and quality are downstream of the broader social determinants of health (income, education, housing, neighborhood, food security) — the social conditions that shape who gets sick in the first place. Why This Framework Matters for the MCAT
When a passage shows a patient with worse care or outcomes, your job is to identify which mechanism is operating: access, affordability, quality, provider bias, or structural factors (communities underserved by design). These distinctions are how the topic is tested.
Quick check: A low-income woman with asthma uses the ER rather than a regular primary care physician. Is this primarily a disparity in access, quality, or outcomes?
Answer: Access — lack of affordable continuous primary care forces reactive emergency use. A classic low-SES pattern that itself drives worse long-term outcomes.
Socioeconomic Status and Health Disparities

Answer: Parsons' sick role — the structural inability to afford time off or care prevents them from fulfilling the role's obligations; the barrier is structural (no paid sick leave), not a failure of motivation.
Common Confusions & Tricks
1. Health disparity ≠ health inequity (but the MCAT may use them interchangeably). If a passage asks about an avoidable, unjust difference, choose inequity. A plain difference in rates is a disparity.
2. Race as a social construct ≠ race irrelevant to health. Race is not a valid biological category for genetic variation, but racism produces real biological harm. Explain race-based differences through social mechanisms, not genetic determinism.
3. Structural racism ≠ individual racism. Structural racism works through policies and systems (redlining, hospital siting) even without individual racist intent. Don't conflate the levels.
4. Gender ≠ biological sex. Disparities can arise from gender norms (men avoiding care), biological sex (drug metabolism), or research bias (women excluded from trials). Know which the passage describes.
5. Intersectionality is multiplicative, not additive. "The intersection of race and gender creates a unique position" beats "she belongs to two disadvantaged groups."
6. Implicit bias ≠ explicit prejudice. Implicit bias is unconscious and can coexist with egalitarian attitudes. The IAT measures associations, not stated beliefs.
7. Minority stress is about excess stress, not stress generally. Stigmatized-group membership itself — discrimination, concealment, internalized stigma — adds burden beyond what non-minority individuals face.
8. The weathering hypothesis is cumulative physiological aging from chronic social stress — most associated with Black women's maternal outcomes, explaining elevated pregnancy risk even at equal SES.
9. "Food desert → structural barrier," not individual choice failure. Answer choices blaming poor diet in low-income communities on individual behavior are typically wrong from a sociological framing.
