What Are Health Disparities?

A health disparity is simply an observed difference in outcomes between groups. A health inequity goes further: it judges that difference to be unjust and avoidable (a normative, ethical dimension). Health equity (everyone having a fair opportunity to be healthy) is not the same as health equality (everyone getting identical resources); equity may require giving more to disadvantaged groups to close a gap.
A boundary to keep straight: this guide is about disparities in health status and outcomes — who gets sick and dies (morbidity, mortality, life expectancy) — not disparities in health care (access to, use of, and quality of treatment, covered in the Health Care Disparities guide). Differences in care are one cause of differences in outcomes, but the two are not the same.
The engine behind most disparities is the social determinants of health (SDoH): non-medical conditions in which people are born, grow, work, live, and age — income, education, housing, neighborhood safety, food access. When a passage asks why one group is sicker than another, think social determinants first. The discipline studying how social conditions distribute health across a population is social epidemiology.
Causes sort along a stream: upstream causes are structural/social conditions (policies, income distribution, segregation, education) far from the body; downstream causes are proximate individual factors (behaviors, biological risk, the disease itself). Effective interventions usually have to move upstream, because downstream fixes leave the structural source intact.
The social gradient of health: health improves continuously with each step up the socioeconomic ladder — not just poor vs. rich, but middle vs. upper-middle too. One of the most replicated findings in medical sociology.

Socioeconomic Status and Health
Must knowSocioeconomic status (SES) is measured by income, education, and occupational prestige. All three independently predict health, and lower SES is robustly associated with higher rates of nearly every disease, shorter life expectancy, and more disability.
Mechanisms Linking Low SES to Poor Health
Know the logic- Material deprivation: low income limits nutrition, safe housing, healthcare, and medications. Food insecurity correlates with both obesity (cheap calories are dense but nutrient-poor) and chronic disease.
- Psychosocial stress: chronic financial/job/neighborhood stress repeatedly activates the HPA axis, raising cortisol. This drives allostatic load — cumulative "wear and tear" from chronic stress that accelerates cardiovascular, metabolic, and immune disease. - Health behaviors: lower-SES individuals have higher smoking, lower activity, less healthy diets — but these are shaped by structural conditions (stress, marketing, food environments), not just personal choice.
- Access to care: more likely uninsured, less likely to have a regular provider, more likely to delay care; plus barriers like transportation, work schedules, and low health literacy (ability to obtain and use health information).
Fundamental cause theory (Link & Phelan): SES is a fundamental cause of disparities because it grants resources — money, knowledge, prestige, connections — that protect health regardless of the specific risk factor of the era. When old threats are solved, high-SES people use resources to avoid new ones, so disparities persist across time.
Quick check: A Medicaid expansion gives low-income adults insurance. Per fundamental cause theory, will SES-based disparities disappear entirely?
Answer: No. High-SES individuals will always leverage additional resources (specialist access, information, connections) to gain a health advantage, even when basic access barriers are reduced.
Racial and Ethnic Disparities in Health
Must knowRace is a social construct — not a biological category — but it is deeply consequential for health. U.S. racial and ethnic minorities experience worse outcomes across many conditions.
Key Documented Disparities
Passage-level- Black Americans: higher rates of hypertension, cardiovascular disease, diabetes, and some cancers. Black infant mortality is more than twice that of white infants, even controlling for income. Black women with college degrees have worse birth outcomes than white women who didn't finish high school — not explainable by education or income alone.
- Latino/Hispanic Americans: the Hispanic (epidemiological) paradox — despite lower average SES, recent Latin American immigrants often have comparable or better mortality than expected, attributed to the healthy immigrant effect and strong social networks. Outcomes worsen with longer U.S. residence (acculturation).
- Native American / Alaska Native: high diabetes, heart disease, and substance use, tied to historical trauma, poverty, and underfunding of the Indian Health Service.
- Asian Americans: heterogeneous — disaggregating reveals wide subgroup variation.
Mechanisms Specific to Race
Know the logic- Structural racism: historical/contemporary policies and institutions that systematically disadvantage minorities — residential segregation (redlining), wealth gaps, environmental injustice, mass incarceration. Systemic, not interpersonal prejudice.
- Discrimination as a chronic stressor: repeated discrimination (overt or microaggressions) activates stress responses, feeding the allostatic load model.
- Weathering hypothesis (Geronimus): chronic racism-related stress causes accelerated biological aging in Black Americans. Explains why younger Black mothers have better birth outcomes than slightly older ones (less accumulated wear), opposite the white pattern. Shows up in telomere length and allostatic load.
- Provider-level bias (recap): unequal treatment at the point of care — e.g., implicit bias yielding less pain medication or fewer cardiac interventions for Black patients — also widens outcome gaps. Quick check: Black newborns have lower survival when delivered by white physicians but comparable or better survival with Black physicians. Which mechanism does this most directly support?
Answer: Implicit bias in clinical care — unconscious biases affect communication, diagnosis, and treatment quality, with race-concordance mitigating some of the effect.
Gender Disparities in Health

The Mortality-Morbidity Paradox

Specific Gender-Based Disparities
Passage-level- Cardiovascular disease: underdiagnosed in women, who more often present with atypical symptoms (fatigue, nausea, jaw pain) rather than classic chest pain; trials historically underrepresented women.
- Pain and mental health: women more often diagnosed with depression/anxiety (and their pain sometimes undertreated as "anxiety"); men more often with substance use and antisocial personality.
- Transgender / gender nonconforming individuals: high rates of depression, anxiety, substance use, and violence-related risk, driven by minority stress (chronic stress of stigma and discrimination).

For health, a poor Black woman doesn't simply experience poverty plus being Black plus being a woman — she occupies a unique position where all three interact, sometimes amplifying disadvantage in qualitatively different ways. This is why higher education does not protect Black women's birth outcomes the way it does white women's: the racial layer cuts across the SES gradient. It also explains why single-dimension interventions often fall short (a "women's health" program ignoring race and class tends to benefit white, middle-class women most).

Answer: It treats all low-income women as homogeneous, ignoring how race, language, neighborhood safety, work schedules, and caregiving differently affect who can actually use the benefit — compounding disparities rather than addressing them.
Geographic and Other Dimensions of Disparity

- Global disparities: the SES-health gradient operates across countries too — high-income nations have longer life expectancy and lower maternal/infant/infectious mortality, while low-income nations bear a disproportionate disease burden. (Country-by-country policy comparison is out of scope.)
- Rural vs. urban: rural Americans have higher rates of the leading causes of death, facing physician shortages, distance to care, and higher poverty.
- Sexual orientation: LGB individuals have higher depression, anxiety, and substance use, explained by minority stress theory (Meyer) — stigma, discrimination, and concealment generate chronic stress. The stigma, not the orientation, causes the disparity.
- Disability status: barriers to physical access, plus diagnostic overshadowing (attributing complaints to the disability) and higher poverty.

Access to Care as a Cross-Cutting Mechanism
Know the logicUnequal access to and quality of care is one cause of the outcome differences this guide describes, cutting across every demographic dimension: insurance coverage, geographic access, cultural competence, health literacy, and provider-level factors (including implicit bias) all shape who gets treated and how well. Historical abuses such as the Tuskegee Syphilis Study also left lasting medical mistrust that depresses care-seeking. Answer: A cultural competence / language access barrier — without effective communication the patient likely didn't understand the prescription's purpose, showing how language barriers undermine care even when the clinic is physically accessed.
Common Confusions & Tricks
Disparity vs. inequity: A disparity is a measurable difference; an inequity adds the judgment that it is unjust/avoidable. To call something an inequity, ask whether it is linked to social disadvantage and preventable.
Race is social, not biological: The MCAT rewards social/structural explanations (discrimination, SES, stress) for racial disparities — not genetic ones, unless the passage specifically indicates a narrow genetic mechanism.
Weathering vs. John Henryism — don't swap them: Weathering (Geronimus) = accumulated biological aging from chronic racism-related stress (cumulative physiological wear). John Henryism = a high-effort coping style linked to hypertension — a behavioral mechanism, not the aging process.
The Hispanic paradox is about immigrants: Better-than-expected outcomes apply mainly to recent immigrants and attenuate with acculturation. U.S.-born Latino individuals do NOT show the same paradox.
Morbidity ≠ mortality: Women = higher morbidity (illness, disability, use), lower mortality (longer life). Men die younger but report fewer illnesses. Know which direction each goes.
Intersectionality ≠ adding disadvantages: It's the interaction of identities producing a qualitatively different experience, not arithmetic addition.
Fundamental cause theory (Link & Phelan) is about resource-based advantage persisting across time — not just poverty. Eliminate one risk factor and high-SES people find new ways to stay healthy, so disparities resist single-point interventions.
Tuskegee and trust: If a passage cites historical medical abuses and present-day consequences, the point is medical mistrust as a barrier to care — the legacy, not an ongoing practice.
